Objectives: Zonisamide (1,2-benzisoxazole-3-methanesulfonamide) is a novel antiseizure medication approved for use in the United States as adjunct therapy in the treatment of partial seizures in adults with epilepsy. It has also been used to treat other conditions including intractable pain. The aim of this study was to determine the usefulness of zonisamide in patients whose seizures were not controlled after having been treated with at least three other currently available anticonvulsant medications or in patients whose pain control was suboptimal despite the use of commonly used drug regimens. Methods: This was a retrospective study documenting the efficacy of zonisamide in 48 consecutive patients who presented at an outpatient neurology clinic at a Veterans Administration hospital. The patients were diagnosed with refractory partial seizures (n = 21) or a variety of intractable neuropathic pain syndromes (n = 27). Results: Sixteen out of 21 seizure patients (76%) experienced a 50% or greater reduction in seizure frequency when zonisamide (1100-200 mg daily) was added to their existing anticonvulsant medication regimen. Of 27 patients with neuropathic pain, 17 (59%) responded to zonisamide, reporting subjective reduction in pain by at least 50%. The most common adverse events were gastrointestinal upset, somnolence, and one case of skin rash. Conclusions: In this study, zonisamide appeared to be an effective adjunct therapy in the treatment of partial seizures in adults who continued to experience frequent episodes while taking other anticonvulsant medications, and in adults whose neuropathic pain was not well controlled with analgesics. These promising results must be tempered by the fact that this investigation included a small patient population in an uncontrolled study design. Further research into the efficacy and tolerability of zonisamide in these areas is warranted.

A family of non-cross-linked and cross-linked copolymers containing decyl, octyl, and hexyl groups as side chains ((CL)-CopolC1-10, (CL)-CopolC1-8, and (CL)-CopolC1-6, respectively) were synthesized by radical-initiated cyclocopolymerization of alkylmethyldiallylammonium bromide monomers without and with a small amount of N,N’-methylenebisacrylamide as a cross-linking agent in aqueous solution. The hydrophobic microdomains of the non-cross-linked and cross-linked copolymers were probed by hypsochromic shifts of the long-wavelength absorption band of Methyl Orange as a solvatochromic agent, noncovalently bound to the macromolecule. The unimolecular decarboxylation of 6-nitrobenzisox-azole-3-carboxylate (6-NBIC), catalyzed by these copolymers in aqueous solution, was used as a model reaction to study the influence of polysoap microenviroment on reactivity. Depending on the hydrophobic group content, (CL)-CopolC1-10 led to a remarkably large rate enhancement, whereas (CL)-CopolC1-8 induced only modest rate acceleration for the decarboxylation of 6-NBIC. A small rate enhancement was observed in the presence of(CL)-CopolC1-6. The decarboxylation rate is also sensitive to changes of the percentage of cross-linking in the macromolecules. A maximum in rate constant was found at about 0.2% (w/w) cross-linking agent for CL-CopolC1-10 and at about 0.4% (w/w) for CL-CopolC1-8 in plots of the rate constant vs cross-linking agent content.

The best dendrimer-risperidone incorporation (46 risperidone molecules per dendrimer) was achieved with a mixture of chloroform: methanol 50:50 v/v solution pH 3. In addition, to explore the possible effects of this complex, in vivo studies were carried out in the zebrafish model. Changes in the development of dopaminergic neurons and motoneurons were studied using tyrosine hydroxylase and calretinin, respectively. Physiological changes were studied through histological sections stained with hematoxylin-eosin to observe possible morphological brain changes. The most significant changes were observed when larvae were treated with free risperidone, and no changes were observed when larvae were treated with the complex.

Introduction of electron-withdrawing substituents on the indole ring likewise increased the rate of substrate consumption but caused a substituent-dependent shift of the site of bond cleavage from the 1,2-isoxazole N-O bond to the C-N bond linking the 1,2-benzisoxazole to the indole moiety. In the case of 3-(2-chloro-3-methanesulfoxylindol-1-yl)-1,2-benzisoxazole, C-N bond cleavage was nearly quantitative, and products resulting from N-O bond reduction were not observed. The overall rates of 3-(indol-1-yl)-1,2-benzisoxazoles reduction were found to be substrate concentration-dependent and observed Michaelis-Menten-type behavior. The apparent V-max of substrate reduction by rat liver microsomes correlated negatively with the free energy of the lowest unoccupied molecular orbitals (E-LUMO) calculated semiempirically using a parameterized model 3 (PM3), and suggested that the initial electron transfer was rate-determining and that the E-LUMO could be used as an indication of the susceptibility of 1,2-isoxazoles to undergo reductive metabolism.
