Zonisamide (1,2-benzisoxazole-3-methanesulfonamide) is a new antiepileptic drug developed in Japan. This compound is insoluble in water, and it is available in tablet and powder form. In experimental animals, this compound has been found to have a strong inhibitory effect on convulsions of cortical origin because it suppresses focal spiking and the spread of secondary generalized seizures. In humans, a series of double-blind, placebo-controlled studies revealed the efficacy of zonisamide for patients with refractory partial seizures and for selected patients with infantile spasms. Its antiepileptic mechanism of action remains unclear, but it is likely to involve blockade of both sodium and T-type calcium channels. Oral bioavailability of zonisamide is excellent in healthy human volunteers. Zonisamide is slowly absorbed and has a mean t(max) of 5 to 6 hours. Almost 100% of it is absorbed; there is no difference in bioavailability between tablets and powder. Zonisamide concentrations are highest in erythrocytes and then in whole blood and plasma. It is approximately 40% to 60% bound to plasma proteins, primarily albumin. Its volume distribution is 0.9 to 1.4 L/kg. In adults, the elimination half-life is between 50 and 62 hours, and it takes as long as 2 weeks to reach steady state. The dose-serum level correlation is linear up to doses of 10 to 15 mg/kg per day, and the therapeutic range is 10 to 40 mu g/ml. However, the relationship between serum zonisamide levels, clinical response, and adverse effects appears weak. Concurrent enzyme-inducing anticonvulsants such as phenytoin, carbamazepine, or barbiturates stimulate zonisamide metabolism and decrease serum zonisamide levels at steady state. Although zonisamide has been reported to increase the serum levels of phenytoin and carbamazepine in some patients, the interactions of zonisamide with other antiepileptic drugs seem to be of minor clinical relevance. A pilot study of zonisamide suppositories revealed that it is beneficial for patients with neurologic disorders in whom antiepileptic drugs cannot be administered by mouth.

ANALYSIS OF SOLVENT EFFECTS ON THE DECARBOXYLATION OF BENZISOXAZOLE-3-CARBOXYLATE IONS USING LINEAR SOLVATION ENERGY RELATIONSHIPS – RELEVANCE TO CATALYSIS IN AN ANTIBODY-BINDING SITE

The mechanisms by which specific solvent properties influence the title reaction, which is extremely medium-sensitive, have been investigated using multiparametric methods. The results of this analysis have been compared with previous experimental studies of the reaction mechanism. Hydrogen-bond donation by solvent and hydrogen-bond donation by tetramethylguanidinium ion in tight ion pairs with the carboxylate greatly retard the reaction. Solvent dipolarity and basicity accelerate the reaction, most likely by helping to break up hydrogen-bonded ion pairs. The rate of decarboxylation in the binding pocket of a catalytic antibody developed for this reaction is slower than that expected of a free carboxylate in an aprotic environment. Therefore, the binding site may contain a hydrogen-bond-donating species. A better catalyst might be developed by approaches that reduce hydrogen bonding in the active site without reducing substrate binding.

Evaluation of zonisamide for the treatment of focal epilepsy: a review of pharmacokinetics, clinical efficacy and adverse effects

Introduction: Zonisamide is a benzisoxazole with 3-methanesulfonamide side chain, chemically unrelated with other anticonvulsants, and approved as mono-therapy of newly diagnosed focal epilepsy with or without secondary generalization in adults or adjunctive therapy in the treatment of partial seizures, with or without secondary generalization, in adults, adolescents, and children aged 6 years and above. Areas covered: Pharmacokinetics, clinical efficacy, and the adverse effects of zonisamide are discussed in the article. The discussion is based on data from published preclinical studies, clinical trials, observational studies, systematic reviews, and approved summary of product characteristics. Expert opinion: Zonisamide is an anticonvulsant with multiple mechanisms of action on neuronal tissue, which achieves seizure freedom in more than 80% of patients with newly-onset focal epilepsy and in 6.2 to 18.1% of patients with focal onset seizures inadequately controlled by first-line anticonvulsants. Within the recommended dose range, it follows linear kinetic of elimination; it is metabolized in the liver by two cytochrome isoforms, so pharmacokinetic interactions are rare and with little clinical significance. Up to 10% of patients taking zonisamide will have problems with weight loss and more than 10% with irritability, confusion or depression, and long-lasting therapy may cause renal calculi in 1.2% of patients.

The proportionality constant is the rate constant for the particular unimolecular reaction. the reaction rate is directly proportional to the concentration of the reactant. RISPERIDONE

Risperidone, a benzisoxazole derivative, is a novel antipsychotic agent that has an extremely strong binding affinity for serotonin 5-HT2 receptors, a strong binding affinity for dopamine D2 receptors, and a high affinity for alpha1- and alpha2-adrenergic receptors and histamine H-1 receptors. Its affinity for serotonin receptors is approximately 200 times greater than that of haloperidol, and its dopamine antagonistic potency is comparable to that of haloperidol. Its major metabolite, 9-hydroxyrisperidone, has similar pharmacologic activity, and thus the parent compound and metabolite form the active antipsychotic moiety Clinical trials demonstrate that risperidone is an effective antipsychotic agent that improves negative as well as positive symptoms of schizophrenia. At recommended dosages, the frequency of extrapyramidal side effects is no greater than that seen with placebo. The drug appears to be an advance in the treatment of psychoses.

Accelerated decarboxylation of 6-nitrobenzisoxazole-3-carboxylate in imidazolium-based ionic liquids and surfactant ionic liquids

We report the use of the unimolecular, spontaneous decarboxylation of 6-nitrobenzisoxazole-3-carboxylate, 6-NBIC, as kinetic probe to investigate the properties of aqueous solutions of a series of ILs, 1-alkyl-3-methyl imidazolium derivatives. The ILs are denoted as [C(4)mim][X], where n indicates the number of carbon atoms in 1-alkyl chain. We studied [C(4)mim][Cl] with X = Cl-, Br-, and BF4-, and the surface-active ILs, SAILs, [C(12)mim][Cl], [C(12)mim][Br], and [C(16)mim][Br]. For comparison purposes we also studied nonmicellizing tetrallcylammonium chloride and bromide, denoted as TRAX, where R is alkyl group and X the anion. We observed a steep increase of values of k(obs) after a certain salt concentration for all the systems used. Electrical conductivity of various aqueous systems was measured, in an attempt to rationalize the kinetic effects. Data from conductivity and kinetic are consistent with the idea that after a certain, high, concentration aggregates of ILs form, and data from the kinetics suggest that water is someway squeezed out from these aggregates. As can be deduced from kinetics, properties of the aggregates formed by [C4mim][X] ILs correlate well with bulk water structure affecting properties of the salts, and seem to have no relation to surface effects.
